To expedite the billing process, please have available for each visit the following information:
- Insurance card with valid ID# and customer service phone number.
- Referral authorization if you are a HMO or POS member.
- Knowledge of what your insurance company covers.
Co-pays, co-insurance and deductibles are an agreement that you have with your insurance provider. These amounts are to be paid at the time of your appointment. Any additional charges not covered by your insurance plan will be billed to you. We accept payment in the form of cash, check or VISA/Mastercard.
